REHABILITATION FOR STATE FUNDS (RESOLUTION 309)
COMPENSATION FROM THE STATE
Child Development Center "Dynamics" provides an opportunity to undergo a course of rehabilitation and correction of children with subsequent reimbursement at public expense.
WHAT ARE THE CONDITIONS?
Every family of Ukraine raising a child with a disability has the right to apply to the state social service of the district where you live with a statement of intention to undergo rehabilitation in the center, which participates in the state reimbursement program under the mechanism "Money follows a person"
Your Route
Collect the necessary documents (IPR, f-027, general documents)
Choose from a list of centers the one that suits you best. Go there and agree on the possibility of rehabilitation
Contact the Social Services of your district, submit documents and write an application for rehabilitation in the selected center. Awaiting funding announcement.
When funding arrives, Social Services will send a request to the center of your choice. The specialists of the center will contact you to agree on the terms and the Rehabilitation Program.
The Social Service and the Center will conclude all the necessary documents
If your rehabilitation does not begin in the near future, but in a few weeks (months) - Be sure to call the Center in advance and agree on a schedule!
Come to class at the appointed time, do not forget to take the necessary documents for initial examinations
MANDATORY QUESTIONNAIRE
If you and your child have chosen the CDD Dynamics and received from the Social Service confirmation of rehabilitation (concluded Agreement) - please fill out the Questionnaire (below) so that we can put your child in line in time and agree with you all important stages and processes of correction course. After filling out the Questionnaire, we will contact you in the next 3 working days, otherwise be sure to call us!
